1. otherwise의 기본 뜻은 other way입니다. wise가 고어로 way를 뜻합니다. 그러니까 여기서는 그냥 다른 방식으로, 즉 두 가족이 친밀해질 법한 상황이었으나 그렇지 않은 방식으로 지내게 되었다는 뜻입니다.
otherwise를 학창시절에 '만일 ~하지 않으면 ~할 것이다'라는 의미로 배우기 때문에 복잡하게 해석하는 경향이 있습니다만 그냥 '다른 방식으로'라고 간단히 해석하면 됩니다.
2. thrust는 과거분사도 역시 thrust입니다. 'benighted hinterland에 thrust away된 상황이었으므로/상황에서'라는 뜻의 분사구문입니다. 두 가족이 thrust away(더 쉽게 cast away라고 생각하시면 됩니다)된 것이므로 주절의 주어가 두 가족이 되어야 맞지만, 주절의 주어와 분사구문의 주어가 일치하지 않는 경우는 많습니다.

For the grudge between the man of herbs and the man of live stock is no new thing; you will find traces of it in the fourth chapter of Genesis.
1번째 질문입니다: otheriwise의 의미 2가지로 갈립니다.
1) 만약 두 가족들이 친밀함이 없다면,
2) 그들의 친밀함을 다른 방식으로(즉, 친밀함 x) 가져왔다
-it has been so once를 아마도 한 때는 친밀했을것이다. 그러나 만약 두 가족들이 친밀함이 없다면 삶의 방식이 친밀함을 가져왔을 것이다(1번해석)
-아마도 한 때는 친밀했을 것이다. 그러나 그들의 삶의 방식이 그러한 친밀함을 다른 방식으로 가져왔다. (싸움을 가져왔다/ 뒷내용이 있음)
2번째 질문입니다.
Thrust away in the benighted hinterland of a scattered market district, 여기의 구문은 어떻게 되나요..? 분사도 아닌거 같고 그래서요.
